Telehealth Consent

This Informed Consent for Telehealth Services describes how healthcare services are delivered through the Vyoni platform, operated by Hydrohealth Solutions LLC. By using Vyoni's services, you acknowledge that you have read and understand this consent.

1. What is telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. For Vyoni's services, telehealth is conducted through asynchronous (store-and-forward) communication, meaning you complete a health questionnaire that is reviewed by a licensed healthcare provider at a later time. In some states, a synchronous (live) consultation may be required by law.

2. How Vyoni uses telehealth

Through Vyoni's platform, you will complete a health questionnaire that includes questions about your medical history, current symptoms, current medications, and other health-related information. This information is transmitted securely to a licensed healthcare provider in our network who will review your submission and determine whether prescription vaginal estrogen & progesterone cream is clinically appropriate for you.

If the provider determines that treatment is appropriate, a prescription will be issued to our partner pharmacy. If the provider determines that treatment is not appropriate, you will be notified and will not be charged.

3. Benefits of telehealth

Potential benefits of telehealth include:

  • Improved access to healthcare services without the need for an in-person office visit
  • Ability to receive care from the privacy and convenience of your home
  • Reduced travel time and associated costs
  • Ability to complete the health questionnaire at your own pace

4. Risks and limitations of telehealth

As with any medical service, there are potential risks and limitations associated with telehealth, including but not limited to:

  • The healthcare provider will not be able to conduct a physical examination, which may limit the provider's ability to diagnose or treat certain conditions
  • There is a risk that the information transmitted may not be sufficient for the provider to make a complete clinical assessment
  • Delays in evaluation or treatment could occur due to technical difficulties or deficiencies in the transmitted information
  • Security protocols could fail, potentially resulting in a breach of privacy of personal health information
  • A lack of access to your complete medical records may result in adverse drug interactions, allergic reactions, or other clinical errors

In some cases, the healthcare provider may determine that telehealth is not appropriate for your situation and may recommend an in-person evaluation with your primary care provider or another specialist.

5. Your rights

You have the right to:

  • Withhold or withdraw your consent to telehealth services at any time without affecting your right to future care
  • Access your medical records generated through telehealth services, subject to applicable law
  • Receive information about the healthcare provider reviewing your case, including their name, credentials, and state of licensure
  • Be informed of all parties who will be present during or have access to your telehealth encounter
  • Expect that all applicable laws regarding patient information and confidentiality apply to telehealth services

6. Privacy and security

All information transmitted through Vyoni's platform is encrypted and handled in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. 7. Prescribing and medications

A valid prescription from a licensed healthcare provider is required before any medication is dispensed. The prescribing provider retains full discretion over whether to issue a prescription. Compounded medications are not FDA-approved. They are prepared by an FDA-registered compounding pharmacy based on the provider's prescription. 8. Emergency situations

Telehealth services are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not rely on telehealth services for urgent or emergency medical needs.

9. Consent

By using Vyoni's services and completing the eligibility questionnaire, you acknowledge and agree that:

  • You have read and understand this Informed Consent for Telehealth Services
  • You understand the risks, benefits, and limitations of telehealth as described above
  • You understand that the healthcare provider may determine that telehealth is not appropriate for your situation
  • You have the right to withhold or withdraw your consent at any time
  • You will provide accurate and complete health information to the healthcare provider
  • You are located in the United States at the time of receiving telehealth services

We are partnered with DrTelx (drtelx.com), an independent telehealth medical group whose providers are licensed in the states where we offer care. We work exclusively with licensed U.S. pharmacies that meet rigorous regulatory and quality standards. Our pharmacy partners include EPIQ Scripts, Foothills Pharmacy, and Emerald Compounding Pharmacy. Every pharmacy in our network is licensed in the states where it dispenses, accredited by the National Association of Boards of Pharmacy (NABP), board-certified, and LegitScript-approved. All prescriptions are filled and shipped by these licensed U.S. pharmacies in compliance with applicable state and federal law.

Our telehealth services are available to patients located in all 50 U.S. states and the District of Columbia. All consultations are conducted by licensed U.S. healthcare providers, and all prescriptions are dispensed by licensed U.S. pharmacies. Treatment is only provided where clinically appropriate and permitted by applicable state and federal law. Availability of specific medications may vary by state based on state regulations and provider licensure. Asynchronous (online questionnaire) care is available in the following 44 states: AK, AL, AR, AZ, CA, CO, CT, DE, FL, GA, HI, IA, ID, IL, IN, KY, MA, MD, ME, MI, MN, MO, MT, NC, ND, NE, NH, NJ, NV, NY, OH, OK, OR, PA, SC, SD, TN, TX, UT, VA, VT, WA, WI, WY.
